Your Breast Augmentation Options
For women who would like to increase the size of their breasts, Dr. Poulter offers both saline (saltwater) filled and silicone gel filled breast implants. Each implant type has its own unique characteristics, and implant selection is largely a matter of individual preference that will be discussed in detail during the consultation.
Saline implants are filled with a sterile saltwater solution. Approved for use in patients age 18 and older, saline implants are inserted while empty and filled once in place, enabling Dr. Poulter to use a smaller incision. Saline implants provide the fullest projection, but they are firmer because they are often overfilled to minimize the risk of rippling. Many younger patients interested in significant enlargement often choose saline but switch to silicone implants later in life.
Dr. Poulter has been using silicone gel implants for reconstruction since 2000 and for augmentation since 2006 after the FDA confirmed their safety and released them for use in the United States. For years, the majority of augmentation patients chose high-profile smooth round gel implants that provided ample enhancement without being in the way of the arm. In March 2013, the FDA allowed the release of the Natrelle® 410 highly cohesive silicone gel implants better known as the "gummy bear" implants for patients age 22 and older.
Dr. Poulter often uses these implants, which are anatomically shaped cohesive implants that mimic the curve and feeling of real breast tissue. They offer an exciting advance in breast augmentation, as they have been shown to decrease the risk of complications, including the development of excess scar tissue (called capsular contracture) and rippling. Additionally, the cohesive consistency of the silicone filling prevents leakage. Their safety and beautiful shape also make Natrelle 410s Dr. Poulter's first choice for breast reconstruction after mastectomy.
The most appropriate placement of the implant is another decision that you and Dr. Poulter will make together. The type of implant selected, as well as your individual anatomy, will influence the implant placement. Generally, implants are placed beneath the chest muscle (submuscular placement) for a more natural appearance. For many patients, Dr. Poulter further refines the procedure using the dual plane placement of implants to correct drooping or an underdeveloped lower pole of the breast. In cases of significant sagging (ptosis), Dr. Poulter may recommend a breast lift in combination with your augmentation.
Dr. Poulter prefers the inferior mammary fold incision for optimal implant placement and surgical precision. The most common incision type, this incision is made in the fold beneath the breast, where it is easily concealed.
